We combine a multidisciplinary team (psychiatric nurse practitioners, licensed clinical social workers and licensed mental health counselors) with diagnostic tools most practices do not offer: Creyos neurocognitive testing, TOVA attention testing, GeneSight pharmacogenetic testing and NeuroStar TMS. Validated outcome measures are tracked at every visit, and the plan changes when the numbers say it should.

The result we are proudest of is a hospitalization rate under 1.5 percent across our entire practice.

It depends on the clinician. Alyssa Plaud Mainelli sees patients from age 10, our therapists see adolescents and adults, and several of our prescribers see adults only. Parents and guardians can complete the intake on behalf of a minor, and our intake team will match your family with the right clinician.

Yes. We provide dedicated, confidential care for first responders and military veterans, including PTSD, trauma, sleep disruption and the cumulative stress of the work. We never contact your department, employer or command.

Openings are often available within two weeks, depending on the clinician and office. We give you the real wait for your clinician when you book.

Psychiatric medication management, individual therapy, comprehensive ADHD testing and treatment, Creyos neurocognitive testing, GeneSight pharmacogenetic testing and NeuroStar TMS therapy.

Our ADHD evaluation goes well beyond a questionnaire. It combines a structured clinical interview, validated rating scales, the FDA-cleared TOVA attention test and Creyos neurocognitive testing, so the diagnosis rests on objective data. TOVA takes about 22 minutes in the office; Creyos takes about 30 to 45 minutes and can be done from home.

The ADHD Testing Package is $649 out of pocket. It includes your intake, the testing itself, and a 30-minute debrief session to walk through the results. HSA and FSA funds are accepted.

GeneSight pharmacogenetic testing shows how your genetics may affect your response to psychiatric medications. The results inform prescribing, so your provider can favor medications more likely to work for you and avoid ones likely to cause problems. It is a simple cheek swab with results in about a week.

Both. Our psychiatric nurse practitioners provide evaluation and medication management, and our LICSW and LMHC therapists provide individual therapy. Many patients do best with both, and because everyone works under one roof, your prescriber and therapist share one plan.

We are in network with Blue Cross Blue Shield, UnitedHealthcare, Aetna, Cigna, Harvard Pilgrim and Tufts Health Plan, plus self-pay and out-of-network options. Coverage can vary by clinician, so we verify your specific plan before your first visit.

If we are out of network for your plan, you are welcome to pay privately. We provide an itemized, coded superbill on request so you can submit it to your plan for reimbursement.

Initial evaluations typically run 50 to 70 minutes. Your provider reviews your history, symptoms and goals, and you leave with a plan and the measures we will track.

Medication management follow-ups run about 15 to 45 minutes depending on your needs. Therapy sessions typically run 30 to 55 minutes. Neurocognitive testing takes about 90 minutes, plus 30 to 45 minutes to review the results.

Yes. Most services, including evaluations, medication management, therapy and follow-ups, are available by secure video anywhere in Massachusetts and New Hampshire. TOVA testing and TMS are in person.

In-person hours vary by clinician and are mostly daytime, with limited evening availability. Telehealth often adds scheduling flexibility. Call us and we will find a time that works.

NeuroStar TMS is an FDA-cleared, non-invasive treatment for depression. Magnetic pulses stimulate the left dorsolateral prefrontal cortex, a mood-regulating region of the brain. It does not circulate through your body the way medication does, so there is no weight gain, sedation or sexual side effects.

Adults with depression who have not had adequate relief from antidepressants, or who cannot tolerate their side effects. NeuroStar is also cleared as an adjunct for adolescents 15 to 21 with depression and for adults with OCD. Your provider evaluates your history and safety screening before recommending it.

Most major plans cover TMS when their criteria are met, usually after one or more antidepressant trials have not worked. Our team handles the entire prior authorization process.

You sit in a comfortable chair while the device delivers pulses that feel like tapping on the scalp. Sessions run about 19 to 37 minutes depending on your protocol. You stay awake, drive yourself home and go back to your day. A standard course is 36 sessions over roughly seven to nine weeks, delivered in our Duxbury office.
