The Therapist Who Explains Why
Most people come to therapy having already been told what to do. Sleep more. Try this app. Just breathe. Joe Giallombardo starts somewhere else, with what is actually happening in your brain and why it learned to work this way.
This is the part of the work he loves. Joe can explain the nervous system, the adaptive brain, and the mechanism behind your particular struggle in language that makes sense the first time, without talking down to you and without turning it into a lecture. People who have spent years collecting advice often say he was the first one who made the advice make sense. That is usually when things start to move.
Joe earned his Master of Social Work at Fordham University’s Graduate School of Social Service and has worked in some of the more demanding corners of Massachusetts psychiatry. What that means for you is a therapist who is hard to rattle. Whatever you bring in, he has sat with a version of it before, and he is not going to flinch, soften it, or hand you a worksheet.
Joe is eclectic on purpose. He incorporates Motivational Interviewing, CBT, DBT, and Acceptance and Commitment Therapy, and which one shows up on a given day depends on you, not on a protocol. He also brings a real interest in narrative and the expressive arts to the work. Not many therapists are equally at home explaining what a brain under stress is actually doing and sitting with someone through the worst week of their life. Joe is.
Before he came to this field, Joe worked in filmmaking, in the wine industry, and in education as a teacher, curriculum designer, and basketball coach. That is not a detour on the way to the job. It is part of why he is good at it. Teaching is where he learned to explain hard things simply. The arts are where he learned how much of healing runs through story, and what changes when the story a person tells about themselves changes. Relationships run through all of it, and they run through his clinical work too.
You’re the expert on your own life, even on the days it doesn’t feel that way. My job is to help make sense of what your brain is doing and why, and then figure out together what to do with that.

What Joe Treats
Choose one to read how he approaches it.
Depression & Low Motivation
Sometimes the problem is not figuring out what would help. It is starting anything at all. Joe works in small, achievable pieces that rebuild momentum, and he checks in with you about whether it is actually working.
Anxiety & Overwhelm
Understanding what your nervous system is actually doing changes how frightening it feels. Joe pairs that clarity with concrete ACT and DBT skills for people whose minds will not stop running.
Trauma & PTSD
Joe starts with a clear explanation of how the brain responds to threat and why the response outlasts the danger. He paces the work to what you can actually tolerate and never pushes past it.
Substance Use & Recovery
Joe treats substance use alongside mental health rather than after it. He uses Motivational Interviewing and mindfulness-based relapse prevention, and he works just as well with people who have not decided anything yet.
Grief & Loss
Joe works with people carrying a loss that still has no room in their life, including the kind of loss nobody gave them permission to grieve.
Relationships & Family Strain
Relationships are the thread running through Joe’s work. He helps adults with family conflict, caregiving strain, and the patterns that keep repeating themselves, and can bring family into the conversation when that helps.
Who Joe Works Best With
Therapy goes better when the fit is right. Here is a straight read on both sides of it.
Joe is probably a good fit if you
- Want to understand the why behind what you are feeling, not just collect coping strategies
- Have tried therapy before and found it too vague, too gentle, or too scripted
- Are carrying something you have been told is too heavy to bring up
- Are dealing with mental health and substance use at the same time
- Are working with a prescriber and want therapy that actually talks to them
- Would rather be treated as a capable adult than as a patient
Joe may not be the right match if you
- Want couples therapy as the main focus. Lauren Schneider specializes in that
- Need a specific manualized protocol such as EMDR or CPT
- Are in acute crisis and need stabilization rather than ongoing outpatient therapy
Not sure? Our intake team will help you find the right person on our team, even if it is not Joe.
Explain It, Then Build It
Understanding First, Then Tools
Joe teaches the mechanism before the intervention. Once you know what your brain is doing and why it learned to do it, the coping strategy stops feeling like homework and starts making obvious sense. He works this way because you are the expert on your own life and the motivation to change has to be yours. He would rather help you find it than hand you a plan.
Eclectic by Design
Joe draws on elements of Motivational Interviewing, CBT, DBT, and ACT as the situation calls for, and runs none of them as a program. He also brings a real interest in narrative and the expressive arts, which is often where things open up for people who found that talking alone never quite reached the thing. He builds tools that hold up in a real week.
Steady With Hard Material
Joe has worked in settings where very little stays hypothetical, so not much is new to him. You do not have to manage his reaction, protect him from the worst of it, or spend six sessions working up to the real thing. You can start where you actually are.
One Team, One Plan
Joe works alongside the prescribers here rather than in parallel with them, so therapy and medication point at the same goals and you are not coordinating your own care or retelling your story at every visit. James Kennedy provides clinical oversight, and Sarah Carey supervises Joe’s clinical work.
